Blood pressure control is the single most important thing you can do to prevent a second stroke. I have tracked mine twice daily for seven years. Here is the monitor I trust and why.
Why BP matters more than anything else
Hypertension is the single largest modifiable risk factor for both first and recurrent strokes. After my hemorrhagic stroke, my neurologist set a target of under 130/80. I have hit that target for six years straight. The only way I know I am hitting it is by measuring at home.
Clinic measurements are snapshots. They are taken in a medical setting, often with white-coat anxiety, and they happen once every few months. Home measurements are a movie. They show trends, morning spikes, evening dips, and the effect of yesterday's salt. If you are serious about preventing a second stroke, you need the movie.
What to look for in a monitor
Not all monitors are accurate enough for medical decision-making. Look for these four features: validated accuracy (validated against a clinical standard like AAMI/ESH), upper-arm cuff (wrist monitors are less accurate), memory storage (at least 60 readings), and an irregular heartbeat indicator (survivors with AFib need this).

How I measure — the protocol
Same time every day. I do morning (before breakfast, after bathroom) and evening (before bed). Same arm. Same posture. No caffeine or exercise for thirty minutes before. I sit with back supported, feet flat, arm at heart level. I take two readings one minute apart and record the average.
I bring a month of readings to every appointment. My doctor uses the home data to adjust medication, not the single clinic reading. The home data has caught medication issues that the clinic missed. It is the most important tool in my prevention kit.
How to read the trends — not just the numbers
A single reading tells you almost nothing. Two weeks of readings tells you something. Six months of readings tells you everything. Here is what to look for in the trend data:
Morning surge: a spike in systolic BP in the first hour after waking — higher than evening readings — is called morning surge and significantly increases stroke risk. It is often invisible without home monitoring. If your morning reading is consistently 15–20 points higher than your evening reading, mention it to your neurologist.
Nocturnal pattern: some monitors have a night mode or can be read with a timestamp. Non-dippers (people whose BP does not drop at night) have higher stroke risk than dippers. CPAP therapy often converts non-dippers to dippers — another reason to treat sleep apnea if you have it.
Medication timing: if your evening readings are much higher than morning readings, your once-daily medication may be wearing off. Your prescriber can time the dose differently or add a second agent. If your reading is highest 30 minutes after taking your medication, that is a false peak and not clinically meaningful. Track both timing and reading.
