The difference in one paragraph

A stroke is a brain injury caused by blocked or bleeding blood flow that lasts long enough to kill brain tissue. A TIA — transient ischemic attack — is the same blockage but the body clears it before tissue dies. The symptoms are identical. The only difference is duration. From the outside, in the first 10 minutes, you cannot tell them apart.

Why "mini-stroke" is the wrong word

Calling a TIA a "mini-stroke" makes it sound minor. It is not minor. About 1 in 3 people who have a TIA will have a full stroke within one year. Almost half of those happen within 48 hours. The TIA is the loudest warning shot the cardiovascular system can fire.

Modern stroke centers treat a TIA as a full medical emergency: rapid imaging, carotid ultrasound, EKG, often admission for monitoring, and immediate initiation of antiplatelet therapy. If a clinician downplays your TIA, get a second opinion.

How TIAs present

Same symptoms as a full stroke: face drooping, arm weakness, slurred speech, vision changes, balance problems, sudden confusion. The difference is they typically resolve in under an hour, often in 5–10 minutes. Survivors describe a "weird episode" they dismissed and went on with their day. That dismissal can be fatal.

What to do if you think you had a TIA

Call 911. Even if symptoms have already resolved. The window to prevent the next event is short and the workup needs to happen now, not at next week's primary care appointment.

In the ER, expect: CT and/or MRI brain imaging, carotid Doppler ultrasound, EKG, possibly an echocardiogram, and blood work. Most centers will start antiplatelet therapy immediately. Some will admit you for 23-hour observation. This is appropriate, not excessive.

After the TIA

TIA recovery is the prevention of the second event. The protocol is the same as post-stroke prevention: BP control, antiplatelet medication, statin if indicated, lifestyle changes, and aggressive treatment of any structural cause found in the workup (carotid stenosis, atrial fibrillation, PFO).

Survivors of TIA who follow the protocol have outcomes nearly indistinguishable from people who never had one. Survivors who dismiss the warning have outcomes that are dramatically worse.

What I wish I'd understood about TIA warning signs before my stroke

My stroke came out of nowhere, or so I thought. At 35, with no obvious risk factors beyond a job that ran on caffeine and adrenaline, the idea of a "mini-stroke warning" was not on my radar. But looking back at my history, there had been brief episodes — a few minutes of unusual heaviness on my left side, a moment of speech that felt strange, a visual disturbance I chalked up to dehydration. I dismissed all of them.

I am not saying I had TIAs before my stroke. I don't know that. But the point is that I had been taught — like most people — to think of stroke as a dramatic, obvious, unmistakable event. So anything subtle got categorized as "not that." That mental model is dangerous.

The research consistently shows that survivors who receive rapid workup and treatment after TIA have significantly better outcomes than those who wait. The FASTER trial showed that dual antiplatelet therapy started within 24 hours of a TIA reduced subsequent stroke risk by about 25% compared to aspirin alone. The window is real. The treatment works. But only if you actually go to the ER.

The ABCD2 score is a clinical tool that predicts your near-term stroke risk after a TIA. Knowing your score helps you understand why your care team is or is not admitting you to the hospital.

ABCD2 score interpretation

Total score: 0–3 is low risk (2-day stroke risk ~1%), 4–5 is moderate (~4%), 6–7 is high (~8%). Many centers admit moderate and high-risk patients for 23-hour observation and initiate dual antiplatelet therapy regardless of score. Ask your team where you fall and what the plan is.

Frequently asked questions

Can a TIA cause permanent damage?

By definition no — if it caused permanent damage on imaging, it is reclassified as a stroke. But repeated TIAs can cause cumulative cognitive effects, and many "TIAs" turn out to be small strokes on MRI.

How long does a TIA last?

Most TIAs resolve within 5–60 minutes. Symptoms lasting longer than that are usually a stroke, not a TIA.

Do you need to be hospitalized for a TIA?

Many stroke centers will admit for 23-hour observation, especially if your ABCD2 score is moderate or high. Same-day rapid TIA clinics are also appropriate.
