Cognitive recovery after stroke follows the same principle as physical recovery: the brain rewires based on what you practice. The games and books below are the tools I use. Here is why they work.
What 'use it or lose it' actually means for the post-stroke brain
Neuroplasticity applies to cognitive function the same way it applies to motor function. The neural pathways involved in memory, attention, language, and processing speed can be strengthened through consistent, challenging use — or weakened through disuse.
The key word is challenging. Doing a puzzle that's easy doesn't push the brain. The difficulty level needs to be just past your current ability — the same progressive overload principle that applies to physical rehab. A crossword you can finish in 10 minutes isn't a challenge anymore. A crossword that takes you 45 minutes and leaves you frustrated is the one that's working.
What to look for in stroke-appropriate cognitive tools
- Adjustable difficulty: your cognitive capacity changes day to day after stroke. A good game or book has multiple difficulty levels.
- Short session length: 15–20 minutes per session. Cognitive fatigue after stroke is real and different from tiredness — it hits fast and recovery is slow. Multiple short sessions beat one long one.
- Multi-domain coverage: memory, language, attention, executive function, and visuospatial processing all deserve dedicated work. A single crossword covers language and memory but misses visuospatial and processing speed.
- Large print: processing deficits and visual field changes make standard-print materials genuinely harder. This is not a preference — it's an accessibility issue.

Two 15-minute sessions per day. Morning and evening. Different tools in each session — one language-focused (crossword, word search), one logic/visuospatial (Sudoku, logic puzzle). This covers more cognitive domains and keeps engagement higher than one long session with one tool.
Track your time per puzzle and your error rate. Progress is not always visible in a week — it shows up over a month. If you're not tracking, you'll underestimate how much you've improved.
Digital vs paper — the honest comparison
Paper puzzles require no device setup, have zero screen fatigue, and are portable. The tactile feedback of writing an answer on paper adds a motor component that is absent from tapping a screen.

Targeted cognitive training (adaptive difficulty, multi-domain, consistent practice) produces measurable improvements in memory, attention, and processing speed in the research literature. Non-targeted easy games produce engagement, not rehabilitation. The two are not the same.
How long does cognitive recovery take after stroke?
Highly variable. The fastest cognitive gains occur in the first 3–6 months, similar to physical recovery. Significant improvement continues in years 2–5 with active practice. Processing speed tends to recover slower than memory. Language (aphasia) has its own trajectory and benefits specifically from SLP work alongside book exercises.
Should I do brain games if I have aphasia?
Yes — but choose the right type. Word-based games (crossword, word search) directly target the language pathways involved in aphasia and are useful with appropriate difficulty. Sudoku and logic puzzles do not require language and work well alongside SLP therapy for aphasic survivors.
